put the date of when the cpr expires, or from what dates its good for. If my cpr shows the date of 10/31/19 a lot of employers believe it expired on that day.

Everything was easy and great to be able to do this online, However, my card is not accepted by my employer since you failed to put my last name on it. My name is Patricia Sarver. You only put Patricia on the card. Can you send a replacement card with both my first and last names please so my employer will accept it. Thanks.
