The question that I got wrong was technically correct. Your literature states that "head wounds usually result in minor injuries and bumps" but the answer that you were looking for was "always call 911." The question did not specify what kind of head injury, therefore I did not click true for "always." You should specify the severity of the injury in the question in order to accurately answer the question.
