SOME QUESTIONS SHOULD HAVE INCLUDED QUALIFIERS. FOR EXAMPLE, IF THE ADULT/ADOLESCENT WAS OR WAS NOT WITNESSED BECOMING UNABLE TO BREATH. ALSO, CONTENT CONTAINED MIS-PHRASING IN A FEW SPOTS (I CANNOT REMEMBER, BUT COULD FIND THEM AGAIN).
