I think you should reword a couple questions, eg #7, in which you want us to perform CPR on someone who's unconscious or non-responsive, but it doesn't mention breathing, and I wouldn't do CPR on someone who is still breathing, even if unconscious and unresponsive (not talking back). Also on #4, I'd add the word "severe," as you can have a mild head injury and not need to go to the hospital or go there without an ambulance. Lastly, instead of "Circulation (compressions)" for C.O.D. question, I'd just state "compressions" or "chest compressions" to avoid confusion.
